For all the attributes of a ketogenic diet (like weight loss), there’s one big side effect you have to be prepared for: the keto flu. The keto flu is a term that refers to the period after you start the diet when your body is adjusting to burning fat for energy. “Some people have no problem with it and others are miserable,” says Mancinelli. In the first week or 10 days, you may feel extremely lethargic in your limbs. Walking upstairs may feel impossible. You may deal with mental fog. Often, keto causes constipation, or potentially diarrhea, because of a change in fiber intake. For that reason, you should pick a start date when your week isn’t crazy with deadlines and obligations; choose a slower time when you can rest as needed. If you are among the minority of people who have a healthy metabolism and good insulin response, then you don't have to worry about your carb intake. Good metabolic health means you don't struggle with some combination of issues with blood pressure, blood sugar, cholesterol, weight, or low energy. Low-carb diets can lead to weight loss, research suggests, since they can help to manage appetite and reduce sources of added calories such as sugar and refined grains. But the primary benefit of keto is restoring metabolic health, improving insulin response, and reducing inflammation, Cucuzzella said. And while weight-loss diets work by restricting calories, Cucuzzella said that could backfire if you're doing keto for the first time, making it tougher to transition to burning fat for fuel if you aren't eating enough overall. Finally, a significant amount of the quick weight loss that people can have on short-term keto diets is water, and those pounds can return when you reintroduce carbs into your diet. Low-carb diets can also be problematic if you have a difficult relationship with food, including current or previous disordered eating habits. These include feelings of anxiety about gaining weight and a compulsive need to monitor or limit food, as well as binge eating, emotional eating, or addictive eating behaviors. Any diet that restricts what you can eat can be extremely triggering for anyone with these or other forms of disordered eating, which experts say can be risky if left untreated. It's best to seek professional support when considering a new diet. It may be tempting to make a dramatic change during a tough time. But if you can't manage your stress, you're not getting enough sleep, or you recently had an upsetting life event, it's the worst time to change your diet, Cucuzzella said. Grains and legumes contribute a significant amount of carbs to the diet. If you eat them while following the keto diet, you risk throwing your body out of ketosis. Keto and paleo diets strongly discourage the intake of added sugars. For both diet plans, this largely falls under their shared message of avoiding heavily processed foods in general. However, paleo dieters are a bit more flexible with this rule, as unrefined sugar sources like honey and maple syrup are still permitted. Keto, on the other hand, doesn’t allow any added sugar sources, refined or not, due to the high carb content of these foods. In line with their shared goal of achieving optimal health, both paleo and keto diets encourage the intake of unrefined, healthy fats. Both diets also recommend moderate-to-liberal amounts of selected refined oils, such as olive and avocado oils, as well as nuts, seeds and fish. Keto places very heavy emphasis on fat in general, as it is the cornerstone of the entire diet. Paleo, while not necessarily a high-fat diet, uses this recommendation to support overall health. One of the primary reasons for the popularity of keto and paleo diets is the notion that they will promote weight loss. Unfortunately, there is limited research available for how effective these diets are for sustained, long-term weight loss. However, some short-term research is promising. A small study of postmenopausal, obese women following the paleo diet showed a 9% weight loss after six months and a 10.6% loss at 12 months. This may have been because a high intake of fat usually leads to a decrease in appetite and fewer overall calories consumed. It may also be that the process of ketosis is leading to more efficient elimination of the body’s fat stores. The exact reason is still unclear. Eat extra fat during the first week to accelerate keto-adaptation. Just be sure to dial fat intake back after the first week or two. Do lots of low level aerobic activity. Walk, hike, jog, cycle, row. Keep things in the aerobic HR zone (under 180 minus age in heart beats per minute), and you’ll increase your utilization of body fat, which will speed up ketone production and adaptation. Eat fiber. Many people on ketogenic diets tend to ignore fiber. That’s a mistake. Fiber doesn’t digest into glucose. It also supports your gut biome. Finally, it’s important to share that, as with any new dietary regimen, it’s important to consult your doctor-particularly if you have an existing health condition. I don’t recommend keto for women who are pregnant or nursing or for teens and young adults who are still growing, unless directed and supervised by a physician. Isn’t The Keto Diet The Same As The Atkins Diet? Many people interpret the Atkins diet to be a low carbohydrate plan, and when I tried the Atkins diet, that’s how I understood it. I counted the grams of carbohydrates I ate but didn’t really pay attention to the protein or fat amounts. Some people replace the carbohydrates they would otherwise be eating with more lean meat (thereby increasing the protein intake but not the fat intake). And unfortunately, eating too much protein is one thing that can prevent your body from getting into ketosis, which is the main benefit of keto. Of course, if you think Atkins stands for a high fat diet, then what you think of as Atkins could be much closer to the keto diet. In general, most people on the Atkins diet don’t do ketone testing to make sure they’re in ketosis, whereas that’s a big part of keto. At that point, the hurdles start to feel insurmountable, she says. What’s the healthiest way to try keto cycling? Despite the concerns, if you want to give keto cycling a try, experts recommend cycling in healthful sources of carbs instead of those craveable, heavily processed refined or sugary carbs. Think sweet potatoes, beans, milk and fruit over muffins, rolls and chips. It’s also a good idea to make a plan for your carb-heavy days so you get the most benefit. For example, Spritzler says you might eat them on a workout day. “Pay attention to how you feel on the days you eat more carbs,” she says. Some questions she suggests asking yourself: “Do your energy levels drop or soar? Are you happier and satisfied or anxious and hungry? Does your sleep improve or worsen? ” Pay attention to your answers. “If you feel better off eating carbs occasionally and it doesn't seem to interfere with your weight, keto cycling may be a good fit for you. These ketones - beta-hydroxybutyrate (BHB), acetoacetate, and acetone - are released into the bloodstream, used by the brain and other organs, and then shuttled into the "energy factory" (aka mitochondria) to be used as fuel (stay with us). What Are the Side Effects? As mentioned, keto breath is a thing, and it's not awesome. Weak bones and stress fractures. Ketones are acidic, and one of their jobs is to pull phosphorous and calcium out of your bones. When ketones are your sole source of energy, you have a buildup, and thus, you deplete your phosphorous and calcium. This can lead to stress fractures and bone problems. Headaches, bad mood, and bad memory. Because carbs are the "optimal energy source for the brain," the keto diet starves your brain of fuel. Lisa said. The result? Fatigue, moodiness, and possible anxiety. Increased risk of disease. The ketogenic diet encourages you to eat an excess of fatty foods - and not the good kind of fat.
Other down sides: There’s an initial period where your body is adjusting to its new carb-free existence, and many people experience symptoms like fatigue, brain fog and nausea for a few weeks. You also end up deficient in important micronutrients, like folate, calcium and potassium, which is why most ketogenic devotees recommend taking multivitamins. Personally, I recommend my clients follow a diet that in its ideal state provides all of the nutrients you need through real, whole foods. Confused on how to eat better? Who is the ketogenic diet right for? If you have health reasons that make you want to try it and eating bacon, eggs and steak salads every day sounds amazing, maybe you could swing it. If nothing makes you happier than a fresh piece of sourdough, or if beans are one of the protein sources you rely on, there’s no point in trying a diet that’s not going to work. For any individual with diabetes, discussing dietary changes - especially those as dramatic as the ones the ketogenic diet requires - with your healthcare team is essential. Such a change may require significant adjustments to medication and insulin to prevent dangerous side effects such as low blood sugar, called hypoglycemia. Outside of physical health changes, one of the biggest concerns of the ketogenic diet may be in long-term adherence. “It’s a very difficult diet to stick to and maintain. Compliance is a challenge because it is so restrictive,” explains Mohr. It's advertised as a weight-loss wonder, but this eating plan is actually a medical diet that comes with serious risks. Other carbohydrates like bread, pasta, rice, potatoes, and wheat-based flour. Anything made with corn, especially high fructose corn syrup and other sweeteners. Legumes like chickpeas and beans of all kinds. These nutrient-rich foods are high in protein and fiber, but the idea with ketosis is to rely more on fat. Many keto dieters limit their protein intake to around 10% to 15% of a day's calories, which means that a single cup of black beans could put you in range of a daily limit. Besides, that cup of black beans also has a hefty dose of carbs. Most fruits. A single apple could put you over your carb count for the entire day. Milk. A cup of whole milk has 12 grams of carbs, while the same amount of whipping cream has less than eight. What do you eat on the keto diet? A typical ketogenic diet consists of 75% fat, 20% protein, and 5% carbs. When you follow the ketogenic diet, your body stops relying on carbs as the main source of energy. This sends your body into a metabolic state called ketosis. Ketosis is when your metabolism burns fat for energy instead of carbohydrates, and produces molecules called ketones in the process. Normally when you eat sugar and carbohydrates, your body turns them into glucose, aka blood sugar. Then, your liver releases insulin, which helps your cells absorb that glucose for energy. But when you limit your sugar and carb intake, there's nothing for your body to turn into glucose. As a result, blood sugar levels remain low, and your body generates ketones, instead. Ketones are acidic, so too many in your body are potentially harmful. However, it's this ketone production that makes the keto diet so helpful for people with certain neurological diseases.

Oddly, the success of ketogenic diets against pediatric epilepsy seems to get conflated by keto diet proponents into suggesting a it is beneficial for everyone. By eschewing carbohydrates, you force your body to burn fat. And indeed, the amount of fat you burn shoots up when you eat a keto diet. At the same time, however, the fat you take in shoots up when you eat a keto diet. What happens to our overall body-fat balance? Body fat loss slows upon switching to the ketogenic diet. On the keto diet, rates of body fat loss may slow by more than half, so most of what is lost is water. If you’re insulin resistant, you might need to start lower, around 20 to 30 grams. Hard-charging endurance athletes can often eat 100 grams or more per day and still achieve ketosis. Everyone approaches keto dieting a bit differently. I recommend tracking total carbs instead of net carbs for ease of tracking, and I don’t worry about the carbs in above-ground green vegetables and avocados. Additionally, some patients may feel a little tired in the beginning, while some may have bad breath, nausea, vomiting, constipation, and sleep problems. Is a ketogenic diet healthy? We have solid evidence showing that a ketogenic diet reduces seizures in children, sometimes as effectively as medication. Because of these neuroprotective effects, questions have been raised about the possible benefits for other brain disorders such as Parkinson’s, Alzheimer’s, multiple sclerosis, sleep disorders, autism, and even brain cancer. However, there are no human studies to support recommending ketosis to treat these conditions. Weight loss is the primary reason my patients use the ketogenic diet. Previous research shows good evidence of a faster weight loss when patients go on a ketogenic or very low carbohydrate diet compared to participants on a more traditional low-fat diet, or even a Mediterranean diet. However, that difference in weight loss seems to disappear over time. A ketogenic diet also has been shown to improve blood sugar control for patients with type 2 diabetes, at least in the short term.|For someone eating 2000 calories per day, that translates to around 167g fat, 75-100g protein, and 25-50g carbohydrates. This is key: the total calories you consume are not NEARLY as important as the overall ratio of protein/fat/carbohydrate that you take in. Consuming foods with this macronutrient ratio causes our bodies to burn fat for fuel instead of carbohydrates, meaning it’s a simple (not easy, but simple) way to burn body fat more efficiently. Because of this, the ketogenic diet has been getting a ton of buzz recently for its weight loss benefits. Many people who adopt a ketogenic lifestyle tout easy weight loss from following the protocol, without adding in extra exercise or worrying about total calories. It’s true, you can experience rapid, significant weight loss if you follow this diet correctly, to do this safely, there are other nutritional considerations - more on that soon. WHERE DID KETO COME FROM? The ketogenic diet is the gold standard diet for epilepsy, and research demonstrates that these benefits could actually extend to other neurodegenerative diseases as well.
The aim of the diet is to keep up the state of ketosis for longer periods, by not eating much carbohydrate. And when we say not much, we mean almost nothing at all. For an effective keto diet, you must cover no more than 5% of your daily calorie intake from carbs. Keto vs low carb diet: which one is better for weight loss and exercising? What do you eat on a keto diet? The tricky part is, for an average person, carbs tend to form the majority of their energy intake and switching from a high-carb diet to a high-fat diet will require some willpower. Good news is, you can gorge yourself on good fats during keto diet, like certain types of cheese and avocados. Also, if you are a fan of seafood, you are up for a treat: foodstuff like clams, oysters and squid contain very little carbs and therefore can be consumed more freely when doing keto.|The Keto Diet, or Ketogenic Diet is a high-fat diet that is moderate in protein and low in carbohydrates. This macro-nutrient ratio allows the body to switch from using carbohydrates (converted into glycogen in the liver) for energy, to using fat (converted into fatty acids and ketone bodies in the liver). The human body is like a hybrid vehicle, relying on either carbohydrates or fat for fuel. For the most part, it cannot use both fuels simultaneously. Consuming fat allows the body to access hundreds of thousands of calories of stored fats that have been locked away during carbohydrate metabolism. In addition to burning fat reserves and super charging weight loss, ketosis produces a clean burning metabolic fuel that has many benefits. Ketones lower production of reactive oxygen species (ROS),1 enhance mitochondrial biogenesis,2 3 and induce positive epigenetic expression.4 Because of its neurological benefits, I went on a Keto Diet in 2014, and it has been helpful in moderating my MS symptoms. The keto-buzz is everywhere you look these days. Nutrition coaches are offering keto plans, meal prep companies have debuted keto food lines, and well-renowned blogs are digging in to what this diet is and why people are so dang curious about it. A lot of celebrities are raving about the benefits they’ve seen from shifting to a ketogenic diet, including a few Kardashians, runway models, actors, and athletes. Naturally, when people in the spotlight adopt a specific lifestyle habit, the rest of society becomes interested to try it out too. As a compliment to our stance on keto, I’m hoping to help clear some noise around the keto diet with an exploration of its history, what (and who) it helps, what the long-term prognosis of a ketogenic diet looks like, and how the diet might fit into your life. FIRST THING’S FIRST: WHAT’S KETO? The ketogenic diet is an extremely high fat and low carb diet in which people methodically consume 75% of their calories from fat, 15-20% from protein, and 5-10% from carbohydrates. Keto is a diet that was developed decades ago and originally used for patients with severe epilepsy, some of whom were on the diet for life with no evidence of harm. It consists of low carb, high fat and only moderate protein intake, as opposed to the Atkins diet. Keto isn’t as meat-heavy as commonly believed. Hwu relies on certain “go to” foods like full-fat regular cream cheese, sour cream and avocados as staples. He also consumes a lot of green vegetables and cauliflower. Since starting the keto diet, Hwu has dropped 25 pounds and has maintained the weight loss. His blood pressure, triglycerides and sugar levels have all decreased, which are healthy signs. His biomarkers, including lipid profile and blood pressure have been excellent, he said. There have been a number of studies that show the connection between obesity and certain cancers. Hwu feels that keto makes sense because his patients are not hungry on it, it manages their weight and blood sugar levels and keeps insulin and IGF-1 levels low - two proteins that have been shown to drive some cancers. “I feel that fat intake has been overly emphasized as a negative factor and that a high intake of carbs and the subsequent spikes in insulin and IGF-1 (an insulin-like hormone in the blood) that they cause are more harmful to health overall,” Hwu said. Hwu’s colleague, Dr. Jennifer McQuade, an assistant professor and physician scientist in Melanoma Medical Oncology at MD Anderson says they are currently conducting both human and animal studies of the effects of diet, including the ketogenic diet, on cancer. In addition, they are testing a plant-based high-fiber diet aimed at the gut microbiome, which has been shown to impact response to immunotherapy, a type of cancer treatment that utilizes the patient’s own immune system to fight the disease. They expect results from the studies early next year. What is the ketogenic diet? The ketogenic diet is a diet that produces reactions in the body similar to those that occur during fasting. This is a type of extreme low-carb diet that was first developed in 1921 due to the ability of this type of diet to reduce or suppress seizures. As new medications to treat seizures were developed, the ketogenic diet became less popular as a way to manage seizure disorders. However, in 2008, a clinical trial showed that a ketogenic diet could help children with treatment-resistant epilepsy become seizure-free. A ketogenic diet is often prescribed for people who have failed two mainline antiseizure drugs, with studies showing seizure-reduction rates as high as 85% after this treatment. It can be effective for patients of any age or seizure type. The reasons why a ketogenic diet works to help reduce seizures are unclear, but it is believed to induce metabolic changes that lower the risk of seizures. The diet itself is a low-carb, high-fat diet that involves extreme reduction of carbohydrate consumption and replacing it with fat, up to a concentration of 70%-80% of calories from fat.
These fats are solid at room temperature while monounsaturated and polyunsaturated fatty acids remain a liquid at room temperature. Just because you’re following a ketogenic diet doesn’t mean you should indulge in every fat you come across. All fats are not created equal. 1: Hydrogenated and partially hydrogenated oils. These fats are found in packaged foods. They increase inflammation and your risk of developing heart disease, cancer, and high cholesterol. If you’re relying on packaged foods to get you through keto, check the label and ditch any foods with these. 2: Highly processed vegetable oils. Corn oil, peanut oil, canola oil, soybean oil, sunflower, and grapeseed oil are all fats that sound healthier than they actually are. 1. Are usually made with genetically modified seeds that are potential allergens. 2. Are cooked well above their smoke point, which makes the oils go rancid. This puts you at risk of developing certain cancers, obesity, and depression. Research shows that cutting off glucose levels with a very low-carb diet makes your body produce ketones for fuel. This change can help to reverse neurological disorders and cognitive impairment, including inducing seizure control. The brain is able to use this alternative source of energy instead of the cellular energy pathways that aren’t functioning normally in patients with brain disorders. A related clinical diet for drug-resistant epilepsy is called the medium-chain triglyceride ketogenic diet, in which MCT oil is extensively used because it’s more ketogenic than long-chain triglycerides. 13) Another dietary therapy for epilepsy called Low Glycemic Index Treatment (LGIT) was developed in 2002 as an alternative to the keto diet. Clinical improvement was observed in Alzheimer’s patients fed a ketogenic diet, and this was marked by improved mitochondrial function. 15) In fact, a European Journal of Clinical Nutrition study pointed to emerging data that suggested the therapeutic use of ketogenic diets for multiple neurological disorders beyond epilepsy and Alzheimer’s, including headaches, neurotrauma, Parkinson’s disease, sleep disorders, brain cancer, autism and multiple sclerosis. To date, there is no research examining the benefits or risks of a vegan keto diet. However, several studies have separately explored the effects of a vegan diet and those of a keto diet. A 2014 randomized controlled trial investigated the effects of a vegan Atkins diet (or eco-Atkins diet). The Atkins diet is similar to a keto diet, as they both restrict carbohydrate intake and cause ketosis. In the 6 month study, participants ate either a low carbohydrate vegan diet or a high carbohydrate vegetarian diet, which included eggs and dairy. These diets comprised 26% and 58% of energy from carbohydrates, 31% and 16% from protein, and 43% and 25% from fat, respectively. This finding indicates that very low carb vegan diets may improve the risk factors for heart disease. Learn more about the difference between the keto diet and the Atkins diet here. Other possible benefits of a vegan diet include a reduced risk of diabetes and some cancers. There are three main reasons to stay away from the keto diet. If the keto diet may help you lose weight or improve certain health markers, why wouldn’t you do it? The answer is simple: It’s very restrictive, so it’s not ideal for long-term for weight loss. 1. It is not sustainable for most people. Asche agrees that the diet is sustainable for some people long-term, but unless you are content with never eating carbs again, it’s just not realistic. 2. It can hurt your relationship with food. The keto diet can also impact your enjoyment of food and how you experience food-centric events, like family dinners, brunch with friends, or happy hour with coworkers. Because keto requires strict adherence that doesn’t allow wiggle room for occasional splurges, it can get in the way of enjoying everyday life. This type of restriction might even make you a bit obsessive, “where you have to track every last gram, macro, never allowing yourself to ‘splurge’ on anything containing carbs or sugar in fear of knocking you out of ketosis,” Asche says.
What is the keto diet? The ketogenic diet is a high-fat, low-carbohydrate diet. It also includes non-starchy vegetables, like brussels sprouts, cauliflower, squash, and many others. The diet limits high carbohydrate foods, including grains, root vegetables, fruits, and sugar. In the early 1920s, medical doctors created the diet as a treatment for epilepsy in children. The word ketogenic means the body produces ketones. Ketones are a type of acid that your body produces when you don’t have enough insulin in your body to use glucose for energy. How many carbs do you eat on the keto diet? Generally, a low-carb diet is when you eat 100-150 grams of carbs per day. The keto diet goes even lower because the goal is to get into ketosis. Most people reach ketosis if they eat 50 grams or less of carbs per day.|Calories still count on the keto diet. However most dieters report reduced appetite and are able to “feel full” while eating less. Feeing full while eating less? Yup, you read that right. Are bananas keto friendly? While bananas are not an “unhealthy” food, their high carbohydrate count (28 net carbs for a small banana!) make them suboptimal for those on a keto diet. If you’re trying out keto and love bananas, consider eating only a small portion of a banana and slicing it very thinly, or substitute bananas for banana-flavored extracts instead. Can I still have cheat days? Yes you can! Many people who are “keto-adapted” (have consistently maintained deep ketosis) report being able to stay in ketosis despite having a rare high-carb treat like beer, sushi or cake. But proceed with caution - “cheating” on a regular basis or designating a weekly cheat day is highly discouraged on keto. Due to the high fat concentration of keto foods you can easily regain the weight (or gain more weight) if you’re not actually in ketosis and cheating regularly. Like all diets - the better you adhere to the diet the better (and faster!) your results. But won’t eating fat make me… Nope! That’s a common misconception. But don’t just take our word for it. Nut butter: Natural peanut, almond and cashew butters. Healthy fats: Coconut oil, olive oil, avocado oil, coconut butter and sesame oil. Avocados: Whole avocados can be added to almost any meal or snack. Non-starchy vegetables: Greens, broccoli, tomatoes, mushrooms and peppers. Condiments: Salt, pepper, vinegar, lemon juice, fresh herbs and spices. Avoid foods rich in carbs while following a keto diet. Bread and baked goods: White bread, whole-wheat bread, crackers, cookies, doughnuts and rolls. Sweets and sugary foods: Sugar, ice cream, candy, maple syrup, agave syrup and coconut sugar. Sweetened beverages: Soda, juice, sweetened teas and sports drinks. Pasta: Spaghetti and noodles. Grains and grain products: Wheat, rice, oats, breakfast cereals and tortillas. Starchy vegetables: Potatoes, sweet potatoes, butternut squash, corn, peas and pumpkin. Beans and legumes: Black beans, chickpeas, lentils and kidney beans. Fruit: Citrus, grapes, bananas and pineapple.
What I didn’t know back then was that my diet was actually making me hungrier. When we mix carbohydrates and fats in the same meal, we actually want to eat more. It’s the reason that you could eat cake or cookies until you’re sick (because they’re full of sugar and fat from butter), but you usually won’t do the same with steak (which is mostly protein with some fat). Remember how I talked about “poor metabolic flexibility” above? Well, if you have poor metabolic flexibility, then eating carbohydrates is almost always going to make you hungry an hour or two after a meal. So while you’re eating more calorie-dense fats on a keto diet plan, most people find that they end up eating less overall (in terms of calories), without trying as hard. And they’re less hungry, which usually also means they’re happier and less moody. That said, the keto diet as a therapy “is not widely accepted, but in my mind, there’s enough data to recommend it,” he says. Krikorian will be publishing research on early Parkinson’s patients with mild cognitive impairment. And while he can’t yet share the results, he says ketosis appears to have multiple effects, including improved brain function and cognition, potentially by way of enhancing energy production and normalizing insulin levels in the brain. One reason reason that low-carb diets like keto may improve cognitive functioning is through reducing insulin secretion in the body. That’s important because research suggests that higher insulin in the circulation correlates with lower levels of insulin in the brain, says Dr. Krikorian. Insulin in the brain is necessary to support memory consolidation; it also correlates with an enzyme that helps clear beta amyloid out of the brain, proteins that form plaques that disrupt brain function as in Alzheimer’s disease, he explains. That said, while it’s a promising area of research, more long-term studies are still need to be done before the keto diet is widely recommended in this disease. What can I drink? Which slimming club is best? Which artificial sweetener should I choose? What is the ketogenic diet? A ketogenic diet is a very low-carb diet, considered to be when you eat a level of carbohydrate of around 30g of carbohydrates per day or below. This encourages the body to get its energy from burning body fat which produces an energy source known as ketones. Note that it is important that you speak to your doctor if you are considering following the diet as precautions may need to be taken before starting. People on insulin will typically require smaller doses of insulin which leads to less risk of large dosing errors. The diet helps burn body fat and therefore has particular advantages for those looking to lose weight, including people with prediabetes or those otherwise at risk of type 2 diabetes. Based on the understanding that carbohydrate is the macronutrient that raises blood glucose the most, the primary goal of a ketogenic diet is to keep consumption lower than that of a traditional low carbohydrate diet with moderate protein and a very high fat content. Q: Does a ketogenic diet help you build muscle? A: No. In fact, a ketogenic diet actually causes your body to lose muscle mass. Research suggests that even if your diet is high in protein, or your protein intake is constant, a carb-restricted diet may promote muscle loss. When we eat carbohydrates with lean protein, our bodies produce insulin. This acts as a sort of key, unlocking our muscles to let the protein in so that it can build muscle tissue. Skipping carbohydrates depletes our energy stores and reduces muscle-building potential. Q: What are the drawbacks? A: In addition to being tough to follow, the ketogenic diet coaxes your body to release more of the stress hormone cortisol. Other down sides: There’s an initial period where your body is adjusting to its new carb-free existence, and many people experience symptoms like fatigue, brain fog and nausea for a few weeks. You also end up deficient in important micronutrients, like folate, calcium and potassium, which is why most ketogenic devotees recommend taking multivitamins. Personally, I recommend my clients follow a diet that in its ideal state provides all of the nutrients you need through real, whole foods. Confused on how to eat better? For any individual with diabetes, discussing dietary changes - especially those as dramatic as the ones the ketogenic diet requires - with your healthcare team is essential. Because carbohydrates are broken down into glucose in the blood, cutting carbohydrates from your diet could cause levels to crash rapidly depending on your current medication regimen. Such a change may require significant adjustments to medication and insulin to prevent dangerous side effects such as low blood sugar, called hypoglycemia. Outside of physical health changes, one of the biggest concerns of the ketogenic diet may be in long-term adherence. “It’s a very difficult diet to stick to and maintain. Compliance is a challenge because it is so restrictive,” explains Mohr.
Recently, many of my patients have been asking about a ketogenic diet. Is a ketogenic diet safe? Would you recommend it? Despite the recent hype, a ketogenic diet is not something new. In medicine, we have been using it for almost 100 years to treat drug-resistant epilepsy, especially in children. In the 1970s, Dr. Atkins popularized his very-low-carbohydrate diet for weight loss that began with a very strict two-week ketogenic phase. Over the years, other fad diets incorporated a similar approach for weight loss. What is a ketogenic (keto) diet? In essence, it is a diet that causes the body to release ketones into the bloodstream. Most cells prefer to use blood sugar, which comes from carbohydrates, as the body’s main source of energy. In the absence of circulating blood sugar from food, we start breaking down stored fat into molecules called ketone bodies (the process is called ketosis). Once you reach ketosis, most cells will use ketone bodies to generate energy until we start eating carbohydrates again. It came from a group of Chinese researchers who presented a study at the American College of Cardiology annual meeting in New Orleans. They found patients who followed a diet low in carbohydrates for nearly two decades had an increased risk for atrial fibrillation. But there are important caveats to consider. While the study suggests a link between the two, it wasn’t a randomized trial, so it doesn’t show a cause and effect, and the participants weren’t on the keto diet. They self-reported their carbohydrate intake and those with lower intake (about 44 percent of their calories - which is higher than the keto diet) were found to have higher rates of the disease. “Only a very limited portion of the data and study was presented at the meeting, but it’s a very loose association,” Chokshi says. The researchers proposed that those at risk were probably eating fewer vegetables, fruits, and grains, which are known for being inflammation-fighting foods.|Lisa said. She noted that these bad things include saturated fat and omega-6 fats from butter, red meat, and cheese. Slowed metabolism. This is the opposite of what you want with a diet, correct? Correct. "The main reason a ketogenic diet is not good for weight loss is that it mimics starvation," Lisa said. In starvation mode, your body clings for dear life to every calorie you come near. As Lisa put it, "this is your metabolism coming to a screeching halt." In addition to a limited diet, bad mood, and missing pasta, you're also destroying your metabolism and putting yourself in a worse place than if you had just been eating carbs all along. The balloon effect. Even worse, once you go back to your normal diet (because as Lisa put it, "a keto diet for life sounds like the worst thing ever"), you will balloon up. Yeah . . . Does It Work For Anyone? While Lisa says there's no reason for the average person to try the keto diet - "science supporting the ketogenic diet is either widely discredited or nonexistent" - it can work for children with epilepsy or someone who has suffered brain trauma or injury. There is some science to back up how a keto diet would work in those instances, but Lisa warns that those people should be under the supervision of both a doctor and a dietitian. Consider where you're getting your information on keto. Is it from social media? Does your source have a degree? Are they versed in nutrition? Make sure you're getting the best information possible - this is your health we're talking about, after all! In Summary . . Lisa said. We're happy to hear it, because we also really like pasta. Modifying the restrictiveness of classic keto can be helpful when starting the diet, or when tapering down to a more sustainable, long term diet. An individualized and structured diet containing highly ketogenic Medium Chain Triglycerides (MCT), allowing for more carb and protein than classic keto. Limits the amount of carbohydrate, encourages fat, and does not limit protein. Carbohydrates are to be accompanied by fat when consumed. An individualized but less structured diet, it uses exchange lists for planning meal and emphasizes complex carbohydrates. It is not intended to promote ketosis. A dietary intervention that shifts the body into ketosis by limiting the window of time one eats during the day, forcing the body to access energy from body fat. Nutritional supplements, electrolytes, hydration and activity levels are also key.
To set up the body for the benefits of keto-adaptation, we consume a healthy, rather than restrictive, amount of calories within the following macronutrient profile that includes ample vegetable intake, moderate protein amount and quality fats with a focus on carb content at around 50 grams for most people. The carb-heavy Standard American Diet is an evolutionary aberration. The constant drip of glucose into our blood is a modern luxury (or, more to the point physiologically-speaking, a modern burden). For most of human history, if we wanted carbs, we had to climb a tree and extricate a bee’s nest, spend hours digging tubers, or wait around for the wild fruit to ripen. We are designed for periods of low food availability, and, especially, low glucose availability. In short, we’re wired to favor fat and ketone burning. Plus, humans are remarkably good at slipping into ketosis. Whereas for most other animals ketosis is difficult to achieve, a human will be mildly ketotic just waking up from a full night’s sleep. So if your goal is weight loss, eat enough fat to be satisfied, but there’s no need to “get your fats in” once you’re satisfied. Drink lots of water. This is especially crucial on a low carb or keto diet. Why? When you eat carbohydrates, your body stores the extra as glycogen in the liver, where they are bound to water molecules. Eating low carb depletes this glycogen, which allows you to burn fat - but it also means you are storing less water, making it easier to get dehydrated. Instead of the traditional recommendation of 8 cups of water per day, aim for 16 cups when following a low carb lifestyle. Keep up electrolytes. The major electrolytes in our bodies are sodium, potassium and magnesium. Because a low carb diet (especially a keto diet!) reduces the amount of water you store, this can flush out electrolytes and make you feel sick (called “keto flu”). This is temporary, but you can avoid or eliminate it by salting your food liberally, drinking broth (especially bone broth), and eating pickled vegetables. People with pancreatic or liver problems, or problems with fat metabolism should also avoid the ketogenic diet. This is because the diet is so high in fat that it puts added pressure on both organs, which are essential for fat metabolism . It is also worth considering that eating a well balanced, keto diet is actually very expensive. For most people, following a low carbohydrate diet, rather than a no carbohydrate diet, is much more practical - as it will also allow for the inclusion of fruit and all vegetables. This represents much better dietary balance and usually leads to people sticking with it for longer. As always with weight loss, in the end it all comes down to taking less energy in than you burn. In the UK, the National Diet and Nutrition Survey says that on average, people get about half of their energy from carbohydrates. So by cutting out the source of half of your energy from your diet - even if some of that energy is replaced by fat - you are likely to reduce your energy intake, which leads to weight loss. But if you can’t sustain the keto diet, don’t worry, you are in the majority. Try considering why you eat, rather than what you eat. Tackling convenience buying and emotional eating is the key to successful weight loss for most people.|There are seven steps to starting a successful targeted keto diet. First, you must follow a standard keto diet for at least four to six weeks. Then, you must determine your unique carb count. Third, minimize carbs. Fourth, eat carbs before or during exercise. Fifth, eat fast-absorbing carbs. Sixth, keep calories constant. 14. How Do I Start the Standard Keto Diet? A successful targeted keto diet starts with first following a standard keto diet for four to six weeks. This ensures your body is fat-adapted and will slip back into ketosis easily after you eat carbs. To start a standard keto diet, keep carbs under 20 grams per day and drink plenty of water. Measure your ketone and glucose levels every day until you have entered ketosis. To kick-start your keto diet, you can fast for 48 to 72 hours and exercise. You won’t enter ketosis until your body has consumed all its stored glycogen. When you exercise, your body burns more energy, and when you don’t eat anything, your body can only get fuel from its glycogen and fat stores.
While fat reigns supreme on the diet, don’t just turn to bacon, cheese, and cream. When choosing your fats, aim to include more anti-inflammatory omega-3s, particularly EPA and DHA, the type that are found in salmon, sardines, oysters, herring, and mussels, says Clevenger. Other healthy fats are a good choice, too; if you haven’t stocked up on avocado, olive oil, and seeds such as chia seeds and flaxseed, definitely do. They’re not only keto friendly - they also offer healthy polyunsaturated and monounsaturated fat that your body needs to perform at its best. With people consuming more sodium than ever in a diet rich in processed food, you’re probably not used to hearing the call to eat more salt. But on keto, it’s necessary. Not only does the clearance of ketones cause the body to lose sodium, but you may be getting much less table salt (which is comprised of 40 percent sodium and 60 percent chloride) now that you’ve kicked out the top source of salt in the standard American diet: packaged, processed foods, including bread, chips, crackers, and cookies. When you consider all of the grain-based foods and sneaky sources of added sugar, it's easy eat a lot more than the recommended amount. Contrary to what social media hashtags would have you believe, there's not much to suggest that it will improve athletic performance. Keto also ranked dead-last (down with another joy-stealer, the Whole 30 Diet) on the U.S. News and World Report's Best Diets list. The lack of research on long-term outcomes, hard-to-follow regimen, and potential health hazards all alarmed the panel of experts. Science simply doesn't support the notion that keto diets keep weight off in the long run, unlike the evidence-backed Mediterranean-style plans. Ketogenic eating may actually increase your risk for kidney and liver problems, plus osteoporosis. Since carb-filled foods contain the highest amount of water and dietary fiber, it's crucial to consider both the immediate side effects (constipation) and future ones (increased risk of GI cancers and decreased immune function) of cutting them out. And they are-just leveling with you here-pretty difficult diets to maintain long-term, both in terms of general enjoyment and food-related sanity. If you're interested in either of these diets (and you are aware that you don't need to go on a diet to have a healthy diet), it's important to know the basics of each, and how they compare to one another. To help you sort out the nuances between Keto and Paleo, we talked with registered dietitian Kristen Kizer, of Houston Methodist Hospital in Texas. What is the Paleo Diet? THE PALEO DIET focuses on foods that are high in protein and rich with fiber. There's a strong emphasis on meat, fruits, and vegetables-basically, anything our ancestors would have consumed more than 10,000 years ago during the Paleolithic era. Because hunter-gatherers like Otzi had limited technology, obviously things like frozen pizza aren't allowed on the diet. The keto - or ketogenic - diet is popular but unsustainable for many people. It involves eating high fat and low-carb foods, with the aim of burning body fat for fuel instead of glucose. It's advertised as a weight-loss wonder, but this eating plan is actually a medical diet that comes with serious risks. In the world of weight-loss diets, low-carbohydrate, high-protein eating plans often grab attention. The Paleo, South Beach, and Atkins diets all fit into that category. They are sometimes referred to as ketogenic or "keto" diets. But a true ketogenic diet is different. Unlike other low-carb diets, which focus on protein, a keto plan centers on fat, which supplies as much as 90% of daily calories. And it's not the type of diet to try as an experiment. Kathy McManus, director of the Department of Nutrition at Harvard-affiliated Brigham and Women's Hospital. How does the keto diet work? Here are the basics of keto: The diet aims to force your body into using a different type of fuel. Instead of relying on sugar (glucose) that comes from carbohydrates (such as grains, legumes, vegetables, and fruits), the keto diet relies on ketone bodies, a type of fuel that the liver produces from stored fat. Simply focus on hitting your protein goal when you do get hungry and eat, but otherwise let your body signal when to eat. 9. Avoid The Keto Flu. Maybe you’ve heard of the keto flu, or maybe you’ve just experienced side effects of starting keto. It’s one of the most common questions about the keto diet for beginners. As your body shifts its primary fuel source from glucose and carbohydrates to ketones and fat, it can take your body some time to adjust to this metabolic change. Ketosis also flushes out a lot of water stored in the body, which can cause your electrolyte levels to dip. How to avoid the keto flu? The good news is, the keto flu is temporary and avoidable. The short version is, make sure you get enough electrolytes (especially sodium, potassium and magnesium) to avoid the keto flu. Salting your food generously is one of the best things you can do here; some people even add sea salt to their water. Let’s look closer at how the ketogenic diet can promote fat-burning and increases energy levels, plus other benefits. 2. increase fat intake. Most of us easily replenish our carb stores by eating fruit, vegetables, grains and legumes, so our carb “fuel tanks” rarely get low and we continue burning glucose for energy. This is why the ketogenic diet is known to support weight loss. Instead of starving yourself, you’re training your body to burn fat for energy. Since carbs are your body’s first choice for energy, the only way to get your body to burn fat for fuel is by getting your body into ketosis. The amount of fat your body can utilize for energy will depend on your body composition and fat percentage. Keeping your body in ketosis for prolonged periods of time teaches your body to burn fat for energy more efficiently, which is how the keto diet can reduce your overall fat mass.|There are several ways to measure ketones in your body. Elevated levels of ketones (the acetoacetate group, to be specific) can be instantly detected in your urine using strips such as KetoStrips. After dipping one of these strips into your urine stream, you’ll be able to find out which stage of ketosis you’re in based on the color guide provided. You can find keto strips at nearly any drugstore and online through Amazon. Ketone breath analyzers allow you to measure your state of ketosis by detecting acetoacetates. A popular brand is Ketonix, which is a rechargeable ketone monitor that can be used over and over again. The ketone blood monitor is the most accurate ketosis testing method. It’s a little more invasive than the other keto testing methods, as it requires a prick of blood from your finger. Test kits are around $40, and blood ketone test strips go for roughly $5 each (you’ll need one for every time you test). How It Works This is the most common approach to keto and involves sourcing 75 percent of calories from fat, 20 percent from protein, and 5 percent from carbs. That means limiting carb intake to about 20 to 30 grams (g) of carbs per day, Shapiro says. It’s important to note that while this is the keto diet that most people follow, it’s not the original, or therapeutic, version of keto that an article in Canadian Family Physician showed can help children with epilepsy. That diet consists of slightly different percentages: 80 percent of calories from fat, 15 percent from protein, and 5 percent from carbs. Risks to Note Kizer says there are a few groups who should not follow the standard version of keto (or any other version either): pregnant women, people with diabetes (at least not before discussing it with a physician), and those with a history of kidney stones. The keto diet boasts a ton of transformation stories-even celebs like Jenna Jameson, Al Roker, and Vanessa Hudgens have dabbled in it (and have the before-and-after photos to prove it). But um, it's still hard work-revamping your diet by almost totally cutting out carbs is a drastic change for your body (especially if you're a pizza and pasta lover), so there's got to be an easier way to get into ketosis (a.k.a., that state where your body burns fat for fuel instead of carbs), right? Maybe with special keto supplements? Hold on, keto supplements? Exogenous ketones or ketone salts (a.k.a. Together, these reportedly work to increase the amount of fat in your body and kick you into ketosis (a.k.a. McWhorter. Ketone supplements also allegedly block carbohydrates from being absorbed or metabolized, says McWhorter. Those claims are, well, just claims-"If you want to waste your money and potentially consume adulterated products with little or no research backing them, then sure, go right ahead and get those keto supplements," says McWhorter. And because there's such limited peer-reviewed research to support using them, they might even be harmful to your health in the long run, says Michelle Milgrim, RD, a nutritionist at Northwell Health in New Hyde Park, New York. But there haven't been any long-term studies looking at these outcomes over time, she adds. Side effects from keto supplements are also a red flag. Well, can keto supplements do anything at all? While keto-specific supplements, like exogenous ketones, do temporarily elevate the level of ketones in the blood, says Amanda Maucere, RD, a nutritionist at Lung Health Institute in Tampa, Florida, the overall impact on the body is not the same as getting there via your dietary choices. This mechanism has served us for hundreds of years through periods of scarcity or famine. A lack of consistent energy slows our metabolism because our body wants to do one thing: keep us alive. A slowed metabolism is also why it’s hard for people who restrict their intake to continue to lose weight past a certain point or maintain their current weight loss long-term. Moreover, there are some confounding variables worth noting when on a ketogenic diet. First, cutting out an entire food group will consequently reduce your intake. Yes, you can eat more protein and fat, but these nutrients are more satiating and less likely to be overeaten than carbohydrates. To put in simpler terms: you could probably eat 3 bagels before you ate 3 steaks or 3 avocados! Additionally, avoiding carbs may also leave you lacking some key nutrients, like the vitamins and minerals found in fruits and vegetables. Furthermore, the weight loss is probably not permanent. The ketogenic diet may help you (temporarily) lose weight, but the odds of it staying off are slim. The idea is that all of that fat-burning will help you lose weight, says Kizer. Is Paleo or Keto stricter? ESSENTIALLY ALL legumes, dairy, refined sugar, potatoes, processed foods, refined vegetable oils, and salt (yes, you read that right), are off-limits on the Paleo Diet. Grass-fed meat, seafood, fresh fruits and vegetables, eggs, nuts and seeds, and certain oils like coconut, avocado, and olive oils. A paleo-friendly dinner might include grilled chicken with steamed vegetables, avocado and fruit. If you think paleo is rigid, keto is even less flexible. Generally speaking, dieters are advised to eat between 20 to 30 grams of carbohydrates per day in order to maintain ketosis. To put this into perspective, a quarter cup of steel cut oats has 29 grams of carbs and a banana has roughly 27 grams of carbs. So if you have a few bites of oatmeal or a small piece of fruit, whoops. That's your carb intake for the day. You can even make this food at home, with little to no chef experience. The plan also provides a list of groceries that you can easily purchase from your local stores. You need not worry about having a limited choice in planning out meals, as there is a wide variety of plans listed in the program that you can choose from. One of the primary features of the program is how they provide you with an 8-week meal plan that is organized according to your physique and choices. You get a diet that is tailored accurately for your body’s calorie intake and activity levels. Recipes that are easy to cook and healthy. These are delicious and simple and do not require one to be a pro at cooking. A set of guidelines that helps you to customize these recipes into essential meals. Several meals plans with a variety of foods so that you will not get bored eating the same thing. Some, like Richard Isaacson, MD, director of the Alzheimer’s Prevention Clinic at Weill Cornell Medicine and NewYork-Presbyterian in New York City, are supportive of low-carb diets. “I believe when people are fueling their brain using ketones or calorie or carbohydrate restriction, it’s a cleaner burning fuel, and a better way to delay brain aging,” he says. Still, the evidence isn’t there to say that the keto version of the low-carb diet is always necessary, or even helpful. Indeed, it may benefit some. For example, a small study published in February 2012 in the journal Neurobiology of Aging followed 23 older adults at a higher risk for Alzheimer’s and found that they experienced better memory function after six weeks on a keto diet. For others with certain genetics, though, it may be harmful, says Dr. Isaacson. For that reason, there’s no one-size-fits-all recommendation. Anyone going on keto needs to have his or her labs taken throughout the process, and do it in conjunction with their treating physician and a registered dietitian, he says. Piles of bacon. An avalanche of fried eggs. These are the promises of keto, the high-fat, low-carb diet steadily increasing in popularity over the past few years. Keto's reputation as a route to rapid weight loss has the diet trending, and with celebrities like Halle Berry and Kourtney Kardashian getting on board, we have a feeling that interest won't be waning any time soon. But what exactly is keto, and is it truly healthy? Okay, what is keto? The ketogenic diet is a high-fat, low-carb eating plan designed to force the body into ketosis, a metabolic state that burns fat for energy. Once in ketosis, the body creates organic compounds called ketones to help create energy lost from those missing carbs. As the body adjusts to running on ketones by burning fat for energy, it's common to experience weight loss. This is the crux of keto's popularity: Many people see results. Still, it's important to note that much of this can be attributed to water weight shed from depleting carb stores, which can easily fluctuate. Staples of the keto diet are fish, meat, eggs, dairy, oils, and green vegetables. Pasta, rice and other grains, potatoes, and fruits are strictly prohibited. Keto works by changing the way the body turns food into energy. Typically, during digestion, we break down carbohydrates - like those found in the verboten foods above - into molecules of fructose, galactose, and glucose, the last of which serves as the body’s primary source of energy. When the body can’t draw it from carbohydrates - either because they’ve been cut out of the diet or because a person hasn’t eaten for a long time - it looks for other forms of energy. The keto diet deliberately places the body in a state of ketosis, where fat is released from cells and turned into ketones, the body’s plan B for energy production. Where did the keto diet start? Most of the body's cells prefer to use blood sugar (glucose) as their main source of energy. The keto diet forces your body to use a different type of fuel. Instead of providing your body with glucose from carbohydrates, the keto diet relies on the liver to break down stored fat into molecules called ketones. For most people to begin using stored fat as fuel, they need to limit daily carbohydrate intake to fewer than 20 to 50 grams depending on body size. But this is a highly individualized process, and some people need a more restricted diet to begin producing enough ketones. It typically takes two to four days to reach a state of ketosis (when fat becomes a main source of energy).|That’s part of the reason people can survive for as long as 73 days without food, because we start to just use the fat stores in our body and we can keep going. If the body thinks this is kind of like itself starving or it’s similar to that, does this actually work for obesity? I don’t believe Vinnie’s photos where he’s all of a sudden got like a beautifully-carved six-pack out there on the streets. But does this help reduce obesity? Could this be, like, a potential cure to America’s obesity epidemic, which I hear is pretty bad, right? The obesity epidemic is serious and Vinnie’s abs are probably attributed to more than the keto diet. He’s probably been to CrossFit. And the data we have so far suggest keto performs a lot like basically every other diet. There are some people who see tremendous success. There are some people who just absolutely miserably fail and maybe even gain weight on the diet.
If you can get a handful of cheese, handful of walnuts or pecans, or add 1-2 tbsp of olive oil, caesar, or ranch dressing to your salad, you’ll get the fat you need. If you can follow those four rules, you should get into ketosis and start losing fat. But to make sure that you’re doing it successfully, it helps to do some degree of tracking. The easiest way is to track your weight. If your weight is going down, you’re probably doing it right. The next easiest way is to get some ketone pee strips. These will change colors based on the level of ketosis you’re in. They’re not perfect, but they’ll give you a rough idea of whether or not you’re getting into keto. The third and best method is to get a blood ketone testing device like I use. This lets you see very clearly whether or not you’re in ketosis, so you know how well you’re sticking to the diet. And what if you use one of these tests and it seems like the diet isn’t working? You’re either not losing weight or not in keto? Make sure you’re really eating no carbs. No sweeteners, no carby dressings or spreads, no high-carb nuts. Reduce your meat intake. You might be having too much meat, so cut it back to one and a half fists. Reduce your total intake. You don’t want to reduce the ratio of fat, so the last thing to try (primarily if you’re not losing weight) is to simply eat less of everything. Follow these five rules, and you should have no problem getting into and staying into ketosis. The first week might be a little rough, but after that you’ll find it’s a surprisingly easy diet to live on.|Most animal products are very low in carbs and ideal for the ketogenic diet. All meat and fish apart from liver and some types of seafood are zero carb. Also, beware of processed meat such as sausages as these foods may contain hidden carbs from added starches and sugar. When it comes to leafy greens, the darker the leaves, the better. Dark leafy green contain very few carbs and are nutrient-dense. Include a variety of greens in your diet, such as spinach, arugula, watercress, Swiss chard, kale, collards, bok choy, lettuce, and beet greens. And keto eating is not all about greens! There are plenty of other low-carb vegetables such as cabbage, cauliflower, Brussels sprouts, zucchini, broccoli, tomatoes, peppers, radishes, daikon, okra, turnips, rutabaga, cucumber, celery, eggplant, asparagus, pumpkin, spaghetti squash, kohlrabi, sea vegetables, and mushrooms.
