Because most folks live on a high carb diet, our bodies normally run on glucose (or sugar) for energy. We cannot make glucose and only have about 24 hours’ worth stored in our muscle tissue and liver. Once glucose is no longer available from food sources, we begin to burn stored fat instead, or fat from our food. Therefore, when you’re following a ketogenic diet plan for beginners, your body is burning fat for energy rather than carbohydrates, so in the process most people lose weight and excess body fat rapidly, even when consuming lots of fat and adequate calories through their daily food intake. Another major benefit of the keto diet is that there’s no need to count calories, feel hungry or attempt to burn loads of calories through hours of intense exercise. This causes the brain to hit the metabolic panic button in demand for a source of fuel. In response, the liver begins to repackage fatty acids into ketones, which are allowed access into the brain for fuel. The rumors are true: You can eat bacon on the keto diet. That seems to be a sticking point for prospective dieters-and for good reason. The ketogenic diet is heralded as one of the strictest eating plans around, but the fact that greasy, fatty strips of meat get a stamp of approval makes it feel sorta-kinda doable. The whole point of going keto is to reach ketosis, a cult-y sounding name for the metabolic process that happens when your body uses fat instead of carbs for energy. To get there, you've gotta do the obvious: eat a whole lot of fat and little to no carbs. The internet is abuzz with anecdotes telling of the amazing benefits of a ketogenic (or “keto”) diet. Nothing new there, then, as every year is marked by the rise and fall of a new diet fad - most amplified by the social media echo-chamber but with little to commend them. This time, though, it caught my attention. Not just because I am a modestly overweight 52 year old whose love for food and drink is slightly stronger than my desire to be skinny, and as a result someone who has been on a diet every January for at least 20 years and now knows he needs he miracle. But because of the science behind it. For a start, I first began reading about ketogenic diets thanks to Ethan Weiss, M.D., a prominent UCSF cardiologist on Twitter, whose scientific acumen I respect. If he believes in the benefits of ketosis, then I should definitely look a little closer. Even after you have finished with doing keto, there is no point going back to your previous - presumably unhealthy - diet. Read on and decide for yourself… What is a keto diet? A keto diet essentially forces your body to produce ketones from fat by throwing you into a state of ketosis. Yup, it really is that simple. Okay, here's the clever bit: in ketosis, your body will use fat for energy, as opposed to carbs, regardless of the source, so it will use fat from the food you eat but also fat that you have stored in your body. Removing glucose from carbohydrate foods will cause the body to burn fat for energy instead. The major differences between the classic keto and the Atkins diet is the former emphasizes healthier keto fats, less overall protein and no processed meat (such as bacon) while having more research to back up its efficacy. In fact, those differences with Atkins outline some of the popular keto diet myths, such as it being another high-protein plan, recommending any type of fat and that barely any science research backs up the benefits. These are nutrition lies, plain and simple. So is the keto diet healthy? If it’s done Atkins style? No. But if relying on healthy fats, greens and organic meats? What does “keto” stand for exactly? Following a ketogenic diet puts your body into a state of “ketosis,” which is a metabolic state that occurs when most of the body’s energy comes from ketone bodies in the blood, rather than from glucose from carbohydrate foods (like grains, all sources of sugar or fruit, for example).
For instance, Weiss himself has been on a low-carb high-fat (though not strictly ketogenic) diet for more than six months, and claims he does feel much better. But he’s clear about what he knows and what he doesn’t. He’s lost weight and his borderline pre-diabetes is gone. “I think I feel great,” he said. But that might be because he’s eating less processed food, sleeping better, or enjoying compliments on his new physique. As to the most exotic claims from health and diet gurus - such as keto diets resulting in euphoria, cognitive boosts, and improvements in anything from kidney function to cancer treatment - “We just don’t have the data on that yet,” said Weiss. The researchers agree that the diet itself isn’t inherently dangerous. But, cautions Weiss, “If you have any medical condition, if you take any medicine at all - there are lots of things that change how medicines work in our bodies, and nutrition is definitely one of them.|A 2012 case study analyzed the anti-convulsant effects of the keto diet on a 26 year old patient experiencing epilepsy. The study showed that the diet improved her symptoms significantly within two weeks of starting the diet in addition to her anti-epileptic medications. When following the keto diet, weight loss can vary from person to person, says Jeff Volek, a registered dietitian and professor at Ohio State University. The initial weight loss is partly due to losing water weight because you tend to retain less water on a low-carb diet. And some studies suggest that you may not continue to lose weight on keto long-term. Some call this the "keto plateau" which is when you stop losing weight altogether. The ketogenic diet isn't necessarily for everyone. Take kids, for example. Nutritionists told Insider that putting children or teens on the keto diet - or any restrictive diet - can lead to nutritional deficiencies and eating disorders. I was still always hungry within an hour after having one. Of course they certainly gave me energy, and I enjoyed the taste, but I did still have hunger. What made all the difference for me, was swapping out the extra fat, like the butter and the MCT oil, and adding protein! Maintaining proper electrolyte balance is important while following a keto diet. Depending on a person’s activity levels, it is recommended that 2 to 7 grams of salt be added to the diet daily. Many people add a cup of high-sodium broth twice a day to meet sodium needs. Would you consider keto a long-term, sustainable diet? A well-formulated ketogenic diet with adequate protein, proper supplements (such as a multivitamin, calcium with vitamin D, phosphorous, and levocarnitine), and regular labs would be sustainable. For reasons not entirely understood even today, fueling the body on primarily ketones reduces seizures. However, with the development of anti-seizure medications, few people with epilepsy rely on ketogenic diets today, according to a 2008 paper in the journal Current Treatment Options in Neurology, but some people who don't respond to medications can still benefit. For weight loss, today's keto diets are the descendants of low-carb diets like the Atkins diet, which peaked in popularity in the early 2000s. Both types of diets reject carbs in favor of meatier meals. There is no single blueprint for the keto diet, but plans usually call for eating fewer than 50 grams of carbohydrates a day. Celebrity adherents to the diet include Halle Berry and Kourtney Kardashian. The ketogenic diet is very low in carbohydrates and very high in fat, putting the body into ketosis-the burning of fat instead of glucose for fuel. In a clinical setting, ketogenic diets limit carbs to 20 to 50 g per day, mostly from non-starchy vegetables. Very low-carb ketogenic diets restrict carbs to 30 g per day. The science stuff: Protein is kept high enough to maintain lean body mass, but low enough to preserve ketosis. Basically you are pushing carbs low enough that your body has no choice but to burn fat for fuel. So a true ketogenic diet can have a ratio of 75% to 90% calories from fat, 10% from protein, and 5% from carbs. But don’t stop reading just yet. In real life, diets labeled as “ketogenic” vary widely. What most people are doing is a modified ketogenic diet or modified Atkins. It doesn’t mean it’s not effective, but it’s not the same thing. Your ketone production may not be the same depending on how you are following the diet. The primary fuel source for this system is creatine phosphate, which your body naturally produces, and which you can increase stores by taking creatine supplements. 20-50% of your body's resting metabolic needs. The glycolytic system uses glucose (a form of carbohydrate) to make ATP. The glucose used in glycolysis can have two fates. First, it can get turned into lactate and exported out of your muscle cells and into your blood. It can also continue into the oxidative system by turning into a molecule called acetyl-CoA and then going through the Kreb's Cycle. Lastly, we have the oxidative system. This system can utilize carbohydrates (as discussed above), fatty acids, or protein to produce energy. It does so by the body converting them into acetyl-CoA first. Your body makes energy in three ways. One way, the oxidative system, utilizes a molecule called acetyl-CoA to do a lot of the work. That is the "starter" to oxidative metabolism. When your body starts relying very heavily on oxidative metabolism, you start producing a lot of acetyl-CoA. Storing glycogen requires water, and once our glycogen stores are depleted (which happens during ketosis), rapid loss of “water weight” can occur. Once you refill the carb deficit, that weight may quickly come back. And as if all that’s not enough, the ketogenic diet can damage your relationship with food. Dieters often only consider the nutritive components of certain diets, while ignoring that food was not only made for energy. Food is rooted in tradition, joy, and satisfaction, and restriction leaves some people with psychological damage. Restriction often leads to an unhealthy preoccupation with food, anxiety around eating, interference with social events and mealtimes, and an increased risk for eating disorders. Overall, there is still a lot of research to be done on the effects of the keto diet. It’s important to be very thorough in researching your eating plan before adhering to something that calls for such extreme measures. A better option? Find an eating style that is sustainable, enjoyable and nourishing. It is possible to find a balance between all of the noise we constantly hear about food. A great way to start would be to try to get in tune with what foods your body is craving and why you might be craving them. Weight is a determinant of health, but it’s not the only one. Our advice is to focus on incorporating wholesome, healthy food while also acknowledging that an indulgent treat is part of life. It’s time we stop adhering to these rigorous rules of diets and begin to develop our intuitive eating skills so we can respect our body and all it does for us, not just judge it for its size.

When choosing your fats, aim to include more anti-inflammatory omega-3s, particularly EPA and DHA, the type that are found in salmon, sardines, oysters, herring, and mussels, says Clevenger. Other healthy fats are a good choice, too; if you haven’t stocked up on avocado, olive oil, and seeds such as chia seeds and flaxseed, definitely do. They’re not only keto friendly - they also offer healthy polyunsaturated and monounsaturated fat that your body needs to perform at its best. With people consuming more sodium than ever in a diet rich in processed food, you’re probably not used to hearing the call to eat more salt. But on keto, it’s necessary. Not only does the clearance of ketones cause the body to lose sodium, but you may be getting much less table salt (which is comprised of 40 percent sodium and 60 percent chloride) now that you’ve kicked out the top source of salt in the standard American diet: packaged, processed foods, including bread, chips, crackers, and cookies. These diets often put people into a state which is called "nutritional ketosis." In this state, blood levels of ketones are elevated slightly, to around 0.5 mmol/L. Meta-analyses have shown that ketogenic diets can be useful for weight loss. However, compared head-to-head with other diets, the ketogenic diet does not appear to be more beneficial for weight loss or fat loss. So it is not the only diet that should be considered. Although it is not superior to other diets for weight loss, a few aspects of a ketogenic diet may provide benefits for some people. For example, there is some evidence that being in a state of ketosis may slightly suppress appetite and allow people better control over their appetite. Exactly why this occurs is not well understood but may result from the ketones themselves or a massive change in dietary composition that promotes a greater feeling of satiety. One of the most prominent features of a ketogenic diet is a rapid drop in water weight with a ketogenic diet.
The aim of the diet is to keep up the state of ketosis for longer periods, by not eating much carbohydrate. And when we say not much, we mean almost nothing at all. For an effective keto diet, you must cover no more than 5% of your daily calorie intake from carbs. Keto vs low carb diet: which one is better for weight loss and exercising? What do you eat on a keto diet? The tricky part is, for an average person, carbs tend to form the majority of their energy intake and switching from a high-carb diet to a high-fat diet will require some willpower. Good news is, you can gorge yourself on good fats during keto diet, like certain types of cheese and avocados. Also, if you are a fan of seafood, you are up for a treat: foodstuff like clams, oysters and squid contain very little carbs and therefore can be consumed more freely when doing keto.|The Keto Diet, or Ketogenic Diet is a high-fat diet that is moderate in protein and low in carbohydrates. This macro-nutrient ratio allows the body to switch from using carbohydrates (converted into glycogen in the liver) for energy, to using fat (converted into fatty acids and ketone bodies in the liver). The human body is like a hybrid vehicle, relying on either carbohydrates or fat for fuel. For the most part, it cannot use both fuels simultaneously. Consuming fat allows the body to access hundreds of thousands of calories of stored fats that have been locked away during carbohydrate metabolism. In addition to burning fat reserves and super charging weight loss, ketosis produces a clean burning metabolic fuel that has many benefits. Ketones lower production of reactive oxygen species (ROS),1 enhance mitochondrial biogenesis,2 3 and induce positive epigenetic expression.4 Because of its neurological benefits, I went on a Keto Diet in 2014, and it has been helpful in moderating my MS symptoms. One concern voiced in some studies is loss of muscle tissue beyond what might be expected from any calorie-restricted weight loss diet, possibly because the body is attempting to break down protein to create glucose, but Volek says this isn't an issue when the ketogenic diet is followed correctly. He says lack of fiber is the biggest concern for keto fanatics, who may experience constipation. But it's possible to get fiber by eating lower-carb vegetables like broccoli and chia seeds. Both diets come with some unpleasant side effects, but Yurechko doesn't advise sacrificing fiber to try keto in the long run. Will Paleo or Keto help you lose more weight? ON PALEO, eating like your ancestors doesn't guarantee you'll lose weight. While the diet emphasizes plenty of foods that are good for weight loss, such as lean protein and fruits and vegetables, you would still need to eat fewer calories to drop a few pounds. So if you're binging on nuts and fruit, you could actually gain weight on the Paleo Diet. While ketosis isn't a magic recipe for weight loss, Konstantinos Spaniolas, Associate Director of the Stony Brook Metabolic and Bariatric Weight Loss Center in New York, says that people successfully lose weight on keto because they tend to eat less. The winner: The Keto Diet. With an important caveat: These results are often short-lived, says Kizer. Is Paleo or Keto the better diet? BOTH KIZER and Yurechko agree that Paleo is the better option in terms of overall health, simply because it's less restrictive and includes plenty of fruits, vegetables, and lean protein. What are the risks of keto cycling? Setting aside intense cravings, there may be other risks of cycling on and off the keto diet. In one short-term, small study, researchers investigated the impact of following a strict keto diet for six days and then going off plan by consuming a drink containing 75 grams of carbohydrate. For reference, that’s slightly less than the amount of carbs in two cans of soda. The damage: “We saw a temporary increase in the levels of endothelial microparticles (sometimes called micro vesicles) in the blood,” explains study co-author and Assistant Professor at the University of British Columbia Jonathan P. Little. Endothelial cells are the single layer of cells that line and protect our blood vessels. “Microparticles are small vesicles that get released from cells when they are inflamed or damaged, so finding an increase in endothelial microparticles tells us that the endothelial cells that line our blood vessels experienced some damage and inflammation when glucose spiked.” Though he says that the increase in markers of blood vessel damage was temporary, the long-term impact remains unclear. Since most people following a western diet are not proficient at metabolizing fat optimally, this period allows the body time to become “fat-adapted”, utilizing dietary fat efficiently and effectively. The ketogenic diet was used in several major U.S. When you start relying on it very heavily, you begin to accumulate acetyl-CoA. At some point, your body starts turning those into a different molecule called ketone bodies, which we colloquially call "ketones." This process of turning things like fatty acids into ketone bodies is called "ketogenesis," and interestingly only occurs in the liver. These ketone bodies then get packaged up by your liver and sent out into the bloodstream, where the rest of your body can use them for fuel. We can return from Biochemistry Town and return to the real world and explore a few things about the ketogenic diet. The first place to start is to go back in time a bit. The brain's metabolism is slightly different from the rest of the body in that it relies much more heavily on glucose metabolism than the rest of the body. While skeletal muscle depends quite heavily on fatty acid at rest, the brain depends quite heavily on glucose metabolism. A ketogenic diet can work wonders for people who are overweight, diabetic and prediabetic or just looking to improve their metabolism. It may not be as suitable for elite athletes or bodybuilders looking to add significant muscle mass. As with any diet, keto will only work if you are consistent and stick with your plan. The powerful health, nutritional, and weight loss benefits of a ketogenic diet have been proven through extensive research. Keto may also improve HDL cholesterol levels, often referred to as “good” cholesterol, better than diets that allow moderate carbohydrate consumption. The loss of extra weight and addition of healthier foods is also a driving force behind improved health. How does your DNA factor in? Unfortunately, not everyone metabolizes fats efficiently enough to experience the benefits of the ketogenic diet. A DNA health test can tell you how efficiently you metabolize fats and if the ketogenic diet is right for you. Staples of the keto diet are fish, meat, eggs, dairy, oils, and green vegetables. Pasta, rice and other grains, potatoes, and fruits are strictly prohibited. Keto works by changing the way the body turns food into energy. Typically, during digestion, we break down carbohydrates - like those found in the verboten foods above - into molecules of fructose, galactose, and glucose, the last of which serves as the body’s primary source of energy. When the body can’t draw it from carbohydrates - either because they’ve been cut out of the diet or because a person hasn’t eaten for a long time - it looks for other forms of energy. The keto diet deliberately places the body in a state of ketosis, where fat is released from cells and turned into ketones, the body’s plan B for energy production. Where did the keto diet start? The keto diet is most assuredly not a fad, at least not in the usual sense of the word. Some, like Richard Isaacson, MD, director of the Alzheimer’s Prevention Clinic at Weill Cornell Medicine and NewYork-Presbyterian in New York City, are supportive of low-carb diets. “I believe when people are fueling their brain using ketones or calorie or carbohydrate restriction, it’s a cleaner burning fuel, and a better way to delay brain aging,” he says. Still, the evidence isn’t there to say that the keto version of the low-carb diet is always necessary, or even helpful. Indeed, it may benefit some. For example, a small study published in February 2012 in the journal Neurobiology of Aging followed 23 older adults at a higher risk for Alzheimer’s and found that they experienced better memory function after six weeks on a keto diet. For others with certain genetics, though, it may be harmful, says Dr. Isaacson. For that reason, there’s no one-size-fits-all recommendation. Anyone going on keto needs to have his or her labs taken throughout the process, and do it in conjunction with their treating physician and a registered dietitian, he says.
Because most folks live on a high carb diet, our bodies normally run on glucose (or sugar) for energy. We cannot make glucose and only have about 24 hours’ worth stored in our muscle tissue and liver. Once glucose is no longer available from food sources, we begin to burn stored fat instead, or fat from our food. Therefore, when you’re following a ketogenic diet plan for beginners, your body is burning fat for energy rather than carbohydrates, so in the process most people lose weight and excess body fat rapidly, even when consuming lots of fat and adequate calories through their daily food intake. Another major benefit of the keto diet is that there’s no need to count calories, feel hungry or attempt to burn loads of calories through hours of intense exercise. In some ways, it’s similar to the Atkins diet, which similarly boosts the body’s fat-burning abilities through eating only low-carb foods, along with getting rid of foods high in carbs and sugar.|When we eat more carbohydrates than we need, our body stores them for later use. Stored glucose is packed tightly into molecules called glycogen which can be used to maintain our body in case we go too long without eating a meal. These reserves can only last about one day; and once depleted, the body must switch to a different fuel source called ketones. Ketones are produced in the liver. In response to low levels of glucose, the body begins breaking down fat tissue for fuel in the form of free fatty acids. Free fatty acids can be used as a fuel source throughout the body except for in the brain. The chart above indicates that the keto diet requires eating macronutrients at a ratio of 60% fat, 30-35% protein and 5-10% carbs. This causes the brain to hit the metabolic panic button in demand for a source of fuel. In response, the liver begins to repackage fatty acids into ketones, which are allowed access into the brain for fuel. Fun fact: Both spinach and avocado contain more potassium than the banana, despite the yellow fruit’s reputation! Magnesium is also super important in your body, especially for sleep and hormone balancing. If you’re having trouble sleeping on Keto, magnesium might be the problem. Magnesium is naturally found spinach, almonds, and avocados, among other foods. Like potassium, it’s just the quantity that can be the problem. 3 cups of spinach is needed to meet your daily magnesium needs, or 6 avocados. For this reason, many people on ketogenic diets choose to supplement. Supplements - I take this kind, epsom salt in the bath works as well. You know this, your body needs water! Water is used to transport the above electrolytes around as well, so if your intake is good but you still feel headachy, swollen, bloated, or don’t have the energy that you would like, chug a big glass of water or two, wait 10 minutes, and see how you feel. It’s a good idea to meet with your doctor or a registered dietitian any time you switch up your diet - whether you’re on keto or another eating plan. What is the ketogenic diet? The ketogenic diet is a diet that is high in healthy fats, moderate in proteins, and low in carbohydrates. There are different variations of low carb, and the keto diet is a special type of low carb with added characteristics. Some people opt to count carbs diligently and some choose to simply focus on eat low carb/keto foods. What are net carbs? Net carbs is simply total carbs minus fiber and non-digestible sweeteners, like erythritol and allulose. We don’t have to count fiber and certain sweeteners in net carbs, because they either don’t get broken down by our bodies, are not absorbed, or are absorbed but not metabolized. Some people on a keto or low carb diet choose to count total carbs instead of net carbs. This makes it more difficult to fit in more leafy greens and low carb vegetables (which are filled with fiber), so you should only try that if you don’t get results with a net carb method. And, start with reducing sweeteners and low carb treats before deciding to do a “total carbs” method. Here are a few things you should know before you start. Yes, you might lose pounds really quickly, but you should also watch out for the side effects or complications. Drastic changes in nutrition can cause headaches, dizziness, weakness and fatigue, aches throughout the body, nausea and other common symptoms of flu. Often, people who stick to a keto diet experience this kind of discomfort, however, the reason behind this is not a virus or infection. The keto flu is caused by the body’s reaction to carbohydrate restriction. Your body is used to using carbs as a source of energy, so drastically reducing them can make you feel lethargic and lifeless. Your body needs some time to adapt to a new eating plan, usually, it takes around 24-48 hours. As you see, keto-flu can feel similar to ordinary flu. You may or may not have all symptoms from the list. Some switch to keto diets so easily that they experience few or no unpleasant symptoms, others feel sick from the first days until the end of the diet but that is not common.|People who follow the targeted keto diet tend to experience the most success when they eat before and during exercise. First, they feel that the extra glucose helps to fuel glycolytic exercises. Second, when performing vigorous exercises, they burn glucose so quickly that they rapidly return to a ketogenic state. However, protein synthesis requires fat and protein, not carbs. 18. Why Should I Eat Fast-Absorbing Carbs? According to the standard keto diet, you should eat low-glycemic, high-fiber carbohydrate sources, like squash, carrots, and blackberries. However, on the targeted keto diet, your goal is to use the carbs immediately to fuel a high-intensity workout. Some people like dextrose, which is found in pre-workout drinks and snacks. A more natural option is potatoes or rice. 19. How Can I Keep Calories Constant? The beauty of a diet that counts macros is that maintaining consistent calories is easy. Get ready for a whole lot of fat, some protein, and just about zero carbs throughout your day. Keto-approved fridges and pantries include plenty of meat, seafood, dairy, eggs, nuts, fats and oils, and some veggies that grow above ground. Plenty of meats: Chicken, pork, steak, ground beef, lamb, bacon, turkey, ham, and sausage (in limited amounts). Fatty seafood: Salmon, snapper, tuna, halibut, cod, trout, catfish, scallops. Shellfish: Crab, clams, oysters, lobster, mussels. Most fats and oils: Eggs, butter, coconut oil, olive oil, ghee, lard, avocado oil (and plenty of avocados!), mayonnaise. High-fat dairy: Heavy cream, soft and hard cheeses, cream cheese, and sour cream. A selection of vegetables: Cauliflower, cabbage, broccoli, zucchini, green beans, peppers, eggplants, tomatoes, asparagus, cucumber, onion, mushroom, spinach, lettuce, and olives. Most nuts: Almonds, peanuts, macadamia nuts, pecans, hazelnuts, walnuts, as well as their retrospective butters (look for natural varieties that aren't sweetened). When the body’s glucose level is reduced due to the diet’s low carbohydrate content, the body acts as if it is in a starvation state - although it is not - and begins burning fats instead of carbohydrates. This process in turn yields chemicals called ketone bodies as an alternative source of fuel. When the body burns ketone bodies, tissue-protective gamma delta T-cells expand throughout the body. This reduces diabetes risk and inflammation, and improves the body’s metabolism, said Dixit, the Waldemar Von Zedtwitz Professor of Comparative Medicine and of Immunobiology. After a week on the keto diet, he said, mice show a reduction in blood sugar levels and inflammation. But when the body is in this “starving-not-starving” mode, fat storage is also happening simultaneously with fat breakdown, the researchers found. When mice continue to eat the high-fat, low-carb diet beyond one week, Dixit said, they consume more fat than they can burn, and develop diabetes and obesity. “They lose the protective gamma delta T-cells in the fat,” he said. Long-term clinical studies in humans are still necessary to validate the anecdotal claims of keto’s health benefits. “Before such a diet can be prescribed, a large clinical trial in controlled conditions is necessary to understand the mechanism behind metabolic and immunological benefits or any potential harm to individuals who are overweight and pre-diabetic,” Dixit said. There are good reasons to pursue further study: According to the Centers for Disease Control, approximately 84 million American adults - or more than one out of three - have prediabetes (increased blood sugar levels), putting them at higher risk of developing type 2 diabetes, heart disease, and stroke. More than 90% of people with this condition don’t know they have it. Obesity and type 2 diabetes are lifestyle diseases. Diet allows people a way to be in control.
How wide is the divide between the hype and the research? Originally developed to treat severe epilepsy in infants and children under medical supervision, today the ketogenic diet is moving to the mainstream as a low-carbohydrate tool for weight loss and as a means to reduce cardiometabolic risk factors-but not without controversy. Today's Dietitian looks at what the ketogenic diet is, what's known about its risks and benefits, and whether patients who say they're "doing keto" are actually following a ketogenic diet. The ketogenic diet isn't just any low-carbohydrate diet, and it's not necessarily similar to the Atkins or Paleo diets. The Atkins diet restricts carbohydrates while emphasizing protein and limiting fat, and while the Paleo diet restricts some carbohydrate-rich foods-primarily grains and pulses-it isn't necessarily low carb. The ketogenic diet is very low in carbohydrates and very high in fat, putting the body into ketosis-the burning of fat instead of glucose for fuel. So if your goal is weight loss, eat enough fat to be satisfied, but there’s no need to “get your fats in” once you’re satisfied. Drink lots of water. This is especially crucial on a low carb or keto diet. Why? When you eat carbohydrates, your body stores the extra as glycogen in the liver, where they are bound to water molecules. Eating low carb depletes this glycogen, which allows you to burn fat - but it also means you are storing less water, making it easier to get dehydrated. Instead of the traditional recommendation of 8 cups of water per day, aim for 16 cups when following a low carb lifestyle. Keep up electrolytes. The major electrolytes in our bodies are sodium, potassium and magnesium. Because a low carb diet (especially a keto diet!) reduces the amount of water you store, this can flush out electrolytes and make you feel sick (called “keto flu”). This is temporary, but you can avoid or eliminate it by salting your food liberally, drinking broth (especially bone broth), and eating pickled vegetables. “We know that these endothelial microparticles are elevated in conditions such as heart disease and type 2 diabetes, which is why we were interested in measuring them,” he explains. Besides the potential for blood vessel inflammation, Little also cautions that the metabolic adaptations your body experiences on the keto diet means that you’re relatively glucose intolerant, so a meal (or day) of high carb eating might not be the best plan. “When you adapt to a ketogenic diet, you become a proficient fat burner and this occurs at the expense of carbohydrate metabolism. It probably isn’t the best thing for your metabolism to throw carbohydrates (especially high glycemic index carbs in high amounts - such as a glucose tolerance test drink that we used or a big 'cheat meal') into the mix if you are on a ketogenic diet,” he says. Devine agrees and shares concerns about the long-term risks of this approach. “The benefits from a ketogenic diet are a one-way street,” she says.

Coconut oil is a saturated fat that people can include, and many people also use medium chain triglyceride oil. People should always stay adequately hydrated. This can also help prevent the side effects of the keto diet.

If you're serious about sticking to this diet and experiencing all of its miraculous effects-including weight loss- remember to devote some time to meal prepping. In the process of learning how to make time for meal prep, I also discovered a handful of new favorite foods. Because let's face it, I couldn't live on eating whole avocados in one sitting and dousing everything in cheese and butter for 10 days. I had to get creative in the kitchen. To boost my daily fat intake, I experimented with foods I've never thought I'd love before going keto. Low-carb picks such as House Foods' tofu noodles (pictured above) and Keto Carne (zero-sugar beef jerky), as well as high-fat snacks such as Cacao Vita's single-origin cacao nibs, became my go-to kitchen staples. For breakfast, I found myself munching on two squares of 90-percent cacao with some almonds (see below) or two tablespoons of Kite Hill Chive Cream Cheese Style Spread (this stuff will make you forget about real cheese!). Just opt for the full-fat version and organic and raw, if possible. But for some people, dairy causes stomach bloating and abdominal pain, a condition referred to as lactose intolerance. For example, a glass of whole milk (which doesn’t have any fat removed) contains 12.8g of carbs per glass. Protein is a vital component of any diet. Protein fuels your body with essential amino acids, helps regulate organs, and builds muscle and connective tissue. Ideally, you should consume 0.8 grams of protein per pound of lean body mass (at least). This will prevent muscle loss and help you build essential neurotransmitters and hormones. To really excel at keto, it’s not just about adding more fat to your diet; it’s also about removing the junk. If you can get a handful of cheese, handful of walnuts or pecans, or add 1-2 tbsp of olive oil, caesar, or ranch dressing to your salad, you’ll get the fat you need. If you can follow those four rules, you should get into ketosis and start losing fat. But to make sure that you’re doing it successfully, it helps to do some degree of tracking. The easiest way is to track your weight. If your weight is going down, you’re probably doing it right. The next easiest way is to get some ketone pee strips. These will change colors based on the level of ketosis you’re in. They’re not perfect, but they’ll give you a rough idea of whether or not you’re getting into keto. The third and best method is to get a blood ketone testing device like I use. This lets you see very clearly whether or not you’re in ketosis, so you know how well you’re sticking to the diet. And what if you use one of these tests and it seems like the diet isn’t working? You’re either not losing weight or not in keto? Make sure you’re really eating no carbs. No sweeteners, no carby dressings or spreads, no high-carb nuts. Reduce your meat intake. You might be having too much meat, so cut it back to one and a half fists. Reduce your total intake. You don’t want to reduce the ratio of fat, so the last thing to try (primarily if you’re not losing weight) is to simply eat less of everything. Follow these five rules, and you should have no problem getting into and staying into ketosis. The first week might be a little rough, but after that you’ll find it’s a surprisingly easy diet to live on.|Most animal products are very low in carbs and ideal for the ketogenic diet. All meat and fish apart from liver and some types of seafood are zero carb. Also, beware of processed meat such as sausages as these foods may contain hidden carbs from added starches and sugar. When it comes to leafy greens, the darker the leaves, the better. Dark leafy green contain very few carbs and are nutrient-dense. Include a variety of greens in your diet, such as spinach, arugula, watercress, Swiss chard, kale, collards, bok choy, lettuce, and beet greens. And keto eating is not all about greens! There are plenty of other low-carb vegetables such as cabbage, cauliflower, Brussels sprouts, zucchini, broccoli, tomatoes, peppers, radishes, daikon, okra, turnips, rutabaga, cucumber, celery, eggplant, asparagus, pumpkin, spaghetti squash, kohlrabi, sea vegetables, and mushrooms. Even fruits have a place in a healthy keto diet. Include low-carb fruits such as raspberries, blackberries, strawberries, blueberries, blackcurrants, lemon, lime, rhubarb, coconut, and avocado. Nuts and seeds are a fantastic source of healthy fats. Macadamia nuts, hazelnuts and pecans are very low in carbs and a great source of monounsaturated fats. Almonds are a good source of protein. Other keto friendly options are walnuts, pine nuts, Brazil nuts, flaxseed, pumpkin seeds, sesame seeds, sunflower seeds, and hemp seeds. Apart from whole nuts, you can use nut and seed butters, coconut, avocado, and cacao butter. Beware of cashew nuts and pistachios as they’re relatively high in carbs. A lack of consistent energy slows our metabolism because our body wants to do one thing: keep us alive. A slowed metabolism is also why it’s hard for people who restrict their intake to continue to lose weight past a certain point or maintain their current weight loss long-term. Moreover, there are some confounding variables worth noting when on a ketogenic diet. First, cutting out an entire food group will consequently reduce your intake. Yes, you can eat more protein and fat, but these nutrients are more satiating and less likely to be overeaten than carbohydrates. To put in simpler terms: you could probably eat 3 bagels before you ate 3 steaks or 3 avocados! Additionally, avoiding carbs may also leave you lacking some key nutrients, like the vitamins and minerals found in fruits and vegetables. Furthermore, the weight loss is probably not permanent. The ketogenic diet may help you (temporarily) lose weight, but the odds of it staying off are slim. The clearer the color, the more hydrated you are. Eating keto-friendly foods rich in fiber can help keep digestive issues like constipation at bay. Fiber promotes a healthy gut by providing bulk and water to your stools, making them easier to pass. Adults should aim to consume at least 25-30 grams of fiber per day. This can be difficult to achieve while going keto as high-fiber foods like whole grains, fruits, and starchy vegetables are limited. While you only have 50 grams of carbohydrates a day on keto, it's important to make sure these carbs count, says Keatley. Not only are you using the bathroom more on keto, but you're going to excrete sodium and potassium through your urine, which are two electrolytes essential for staying hydrated. Depending on the type of exercise you are doing, the activity can exacerbate keto-flu symptoms like fatigue and digestive issues. That's because our bodies are used to pulling energy from carbohydrates during an intense activity like HITT workouts or running, says Keatley. While you're transitioning to ketosis, opt for low to moderate activities like walking or leisurely biking as your body adjusts to a new fuel source. This may help you avoid uncomfortable symptoms associated with the keto-flu, like digestive issues. Consider starting with a less intense moderate- to low-carb diet or making small keto-friendly swaps to your normal meals. The keto-flu is a natural response to switching your primary energy source from carbs to fats. Not everyone will experience keto-flu symptoms, but those that do usually experience them in the first few days of starting the diet and begin to feel better within a week. The keto diet is not for everyone and can cause health issues, like nutrient deficiencies, if it's not done properly. For this reason, consider consulting a registered dietitian or health professional before going keto. Even the USDA has a definitive food database which can help get you started. Admittedly, the keto diet can be a bit difficult to follow in the long run. That’s mostly thanks to how different it is than the diet that most Americans are used to. As a result, there are several different types of ketogenic diets you can choose from, ranging from super restrictive to somewhat forgiving. A good rule for starting out is to try not to go over 20 g of net carbohydrates a day. Net carbohydrates: Fiber is considered a carbohydrate, but it isn’t digestible. So we can take total carbohydrate in a food and subtract out fiber, and we are left with net carbohydrates. Vegetables and nuts often are high in fiber, so they may look like they are high in carbohydrate, but nearly half of the carbohydrates are fiber, so they’re not digested by the body.|This can lead to a variety of chronic health conditions including heart disease, diabetes and high blood pressure. And since it nixes entire food groups - including heart-healthy whole grains and nutrient-rich fruits - followers usually come up short on critical nutrients. To add insult to injury, the diet’s high fat content and lack of fiber can lead to constipation (among other ailments). Q: Who is a candidate for the keto diet? A: As the research stands now, the keto diet is only appropriate for people with epilepsy. But the only way to tell if your body is in ketosis is a blood test. “Because the keto diet is so restricted, you’re not receiving the nutrients - vitamins, minerals, fibers - that you get from fresh fruits, legumes, vegetables and whole grains,” says Dr. Ring. How It Works This is the most common approach to keto and involves sourcing 75 percent of calories from fat, 20 percent from protein, and 5 percent from carbs. Because the keto diet helps to reduce blood sugar and insulin levels, it is now being used for people with type 2 diabetes and for people with prediabetes to lower their hemoglobin A1C to a healthy level. Who else can benefit from this diet? Women with polycystic ovary syndrome who follow a low-glycemic-index diet - a variation of the keto diet that involves eating based on how foods affect your blood sugar level - have also seen positive results. Recent studies show the diet may treat the following alone or have compounding effects with other medical treatments: migraine headaches, brain tumor/cancer, Alzheimer’s, Parkinson’s disease, mitochondrial disease, Lou Gehrig’s disease (ALS), autism, and traumatic brain injury. Overall, more research is necessary. For the general population, is the keto diet healthy? While this diet has been shown to accelerate weight loss and to improve certain health conditions, it has its fair share of critics. Make sure the bulk of your diet still comprises nutrient-dense whole foods whether or not you choose to include sugar alcohols in your keto diet. Is coconut sugar keto? For all intents and purposes, coconut sugar is the same as regular white sugar in the context of a keto diet. They contain similar carbs and calories per serving. The glycemic index of coconut sugar is a bit lower but not enough to really matter. That said, a small amount of sugar, coconut or otherwise, won’t automatically kick you out of ketosis. One teaspoon of sugar has 4 grams of carbs. I add a teaspoon of sugar to my coffee each morning with no ill effects. Is it the best use of your carb allowance from a nutrition perspective? No, but if it’s the sensible indulgence that allows you to reap maximum enjoyment from your keto diet, don’t sweat it. Can you build muscle on keto? Absolutely. Plenty of folks successfully build and maintain muscle mass on a ketogenic diet. Because keto is so nutrient-dense, there just will be less bulk, including water, in your digestive tract. Less bulk in your digestive tract will mean an immediate reduction in the size of your midsection. Those with more to lose will see more results faster, as far as fat loss goes, on the ketogenic diet. Weight loss will be drastic at first, and will slow as you approach your goal weight. For example, a 300-pound 5’5″ 40-year-old female will have a much larger energy requirement (2438 calories) to maintain that weight. If she eats 1400 calories, she will lose an average of 2 pounds a week. If she exercises, even walking for 20 minutes after dinner, because her body has to expend more energy to move than a person who is 125 lbs, she will lose even more. For a person who is trying to lose ‘the last 10 pounds’ losing 1 pound (that’s 4 cubes of butter and nothing to discount!) a month isn’t an unrealistic expectation.|That’s a huge change! By reducing carbs and replacing them with fat, we force the cells of our body to use ketones for fuel instead of glucose. I like to think of it as selecting high-octane over regular fuel for your body. The focus of the diet is changing the fuel source, not necessarily calorie restriction, which is the basis for most other weight-loss diets. This process results in weight loss, improved brain function & performance, and just an overall feeling of wellness - both physically and mentally! Ketones are responsible for a lot of the keto benefits you might have heard about, like fewer cravings, more brain power and lasting energy. The keto diet is one way to get your body to make ketones. Your body can also produce ketones when you’re intermittent fasting or taking keto supplements like Bulletproof Brain Octane C8 MCT Oil, aka the most ketogenic MCT oil. Ketosis delivers a bunch of health benefits besides just burning fat. Your metabolism works differently on keto, and people report the following changes in their mind and body. More than 60% of your brain is fat, so it needs a steady supply of fat to keep the engine humming. The quality fats you eat on a ketogenic diet do more than feed your day-to-day activities-they also feed your brain. When your body uses ketones for fuel, you won’t experience the same energy crashes or brain fog as you do when you’re eating a lot of carbs. The idea is that all of that fat-burning will help you lose weight, says Kizer. Is Paleo or Keto stricter? ESSENTIALLY ALL legumes, dairy, refined sugar, potatoes, processed foods, refined vegetable oils, and salt (yes, you read that right), are off-limits on the Paleo Diet. Grass-fed meat, seafood, fresh fruits and vegetables, eggs, nuts and seeds, and certain oils like coconut, avocado, and olive oils. A paleo-friendly dinner might include grilled chicken with steamed vegetables, avocado and fruit. If you think paleo is rigid, keto is even less flexible. Generally speaking, dieters are advised to eat between 20 to 30 grams of carbohydrates per day in order to maintain ketosis. To put this into perspective, a quarter cup of steel cut oats has 29 grams of carbs and a banana has roughly 27 grams of carbs. So if you have a few bites of oatmeal or a small piece of fruit, whoops. That's your carb intake for the day. For everyone else, carbs are still probably your best bet, especially if you’re running at higher intensities than you would be when running 50 miles at a 15-minute/mile pace. “When you’re running at a lower intensity, you need to burn more fat as fuel,” Kasparek explains. Which brings us back to the main question: Does the keto diet work and should you try it? The bottom line is: If done right, runners who need to be able to tap into fat stores over long distances may benefit. But that’s not to say those benefits can’t be reaped by following a low-carb, high-fat diet that doesn’t put your body into ketosis instead. Runners who enjoy shorter distances at faster paces or who incorporate high-intensity workouts into their training are still better off eating a healthy, balanced diet that includes carbs. For weight-loss goals, Kasparek says, it’s possible to lose weight by tweaking a regular diet, say, by adding more healthy fat or cutting out processed foods. But there’s more than one way to lose weight, and one banana isn’t going to set you back-we promise. The report goes on to say that while these various diseases are clearly different from each other, the ketogenic diet appears to be so effective for neurological problems because of its “neuroprotective effect” - as the keto appears to correct abnormalities in cellular energy usage, which is a common characteristic in many neurological disorders. In mouse models, a study showed that a keto diet could slow disease progression for both ALS and Huntington’s diseases. In fact, more than one animal study has discovered a potential benefits of the low-carb, high-fat diet or intermittent fasting in delaying weight loss, managing glucose and protecting neurons from injury. Interestingly, it’s also been shown to slow disease progression in mouse models of both ALS and Huntington’s diseases. Researchers believe that the ketogenic diet can also help patients with schizophrenia to normalize the pathophysiological processes that are causing symptoms like delusions, hallucinations, lack of restraint and unpredictable behavior. One study found that the keto diet lead to elevated concentrations of kynurenic acid (KYNA) in the hippocampus and striatum, which promotes neuroactive activity. About half said that they were and subsequently began a strict low-carbohydrate, high-fat routine. The other men and women continued with a high-carbohydrate diet. Everyone’s meals were matched in terms of how many calories, relative to body weight, they ate. Before the diets kicked off, though, the researchers drew blood from the athletes before and after a workout, to establish their baseline bone health and other markers of their health and fitness. Bone is an active tissue, constantly breaking down slightly and remodeling itself in response to the demands we place on it. Knowing this, the researchers checked for the levels of specific substances in the athletes’ blood known to be associated with bone breakdown, rebuilding and overall metabolism. Then the athletes embarked on three and a half weeks of intense training, while eating mostly fat or mostly carbs. Afterward, the researchers again drew their blood and rechecked the markers of bone health. They found differences. The markers of bone breakdown were higher now among the athletes on the keto diet than at the start of the study, while those indicating bone formation and overall metabolism were lower. These same markers were generally unchanged in the high-carb athletes. The athletes on the ketogenic diet, in other words, showed signs of impaired bone health. How their bones might have been affected by their eating is still unclear, says Louise Burke, the head of sports nutrition at the Australian Institute of Sport in Canberra and one of the lead authors of the new study. “We believe that the keto diet may affect bone metabolism due to the downstream effects of low-carbohydrate availability on certain hormones, along with other factors,” she says. But more study, of course, is needed.
If you haven’t had 3 balanced meals with protein, fat, and a vegetable every day, you aren’t going to feel good. You cannot live on keto brownies and think that you will benefit from the mental clarity and increased energy from the ketogenic diet. Third, if you have given keto a solid chance, with 2-3 balanced meals a day, no artificial sweeteners or franken foods, and you’ve supplemented with electrolytes as described below, AND you’re drinking at least 3 quarts of water a day for a total of 2-3 weeks, it may be that keto just doesn’t work with your body - either at all, or at this time in your health. You may have an underlying health issue, or your body just may prefer less rich food. I know it happens! People on the ketogenic diet need to supplement with potassium and make sure you’re consuming salt. Magnesium is another electrolyte that most people supplement, and some people get from taking epsom salt baths. Unlike the Atkins Diet, which is high in protein, a keto diet avoids eating too much protein. In the past, people thought large amounts of protein can turn into glucose in a process called gluconeogenesis, which takes you out of ketosis. More recent findings suggest the possibility of gluconeogenesis isn’t as big a problem as we once believed, but on a keto diet, the majority of your calories should come from quality fats-not protein. There’s a bit of a learning curve when you’re finding out what to eat on keto. Broadly speaking, it’s best to get your dietary fat from nutrient-dense, whole food sources. That means eating more foods like avocados, coconut oil, olive oil and butter (or Bulletproof Grass-Fed Ghee). Your protein intake should primarily come from fatty cuts of protein like salmon and, yes, bacon. How long does it take to get into ketosis? It can take anywhere from 2-3 days to a few weeks to enter ketosis, depending on your body’s ability to adapt to burning fat for fuel and increasing your ketone levels.
